How to Make and Cancel an Appointment

This is a short online form and is the quickest and simplest way for the majority of our patients to access our clinical team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.
Please do not use this online form for urgent or emergency requests.
- The submitted forms will only be read during office hours.
- The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays.
- They will be read Monday-Friday between the hours of 08:00-18:30.
- Please contact 111 during out of hours or 999 if this is an emergency.
Book face-to-face or remote appointments with your GP, nurse or clinician at a time that suits you.
Your details and information are protected by the highest standards of online security, so all you need to worry about is what to do with the spare time you’ve earnt.

Appointments
Please attend or telephone the surgery at any time during working hours to make an appointment to see the doctor, nurse or other member of our clinical team. Patients have the right to express a preference of GP. Patients will be offered an appointment within 48 hours with a GP if required and can also book appointments up to 8 weeks in advance.
Urgent Appointments
Always ring surgery as early as possible. An emergency appointment will be offered for as soon as possible.
Online Appointments
You now book appointments via the patient access online system. You will need a registration letter to do this, please provide your ID and ask our receptionist to print one for you, then use the link below to register and login.
Be On Time
Please arrive promptly and remember to cancel your appointment if you can no longer keep it; someone else may need to see the doctor urgently.
Cancelling Appointments
The practice expects patients to keep appointments. Always let the surgery know if you cannot attend your appointment as another patient may have an urgent need. Tel: 020 8655 4013 or 01689 849 993
